SQA membership is based on a calendar year operating from 1 January through 31 December and membership dues are not prorated.

Resume documenting active involvement in the QA profession, including all of the following:
- At least two years current professional experience in QA;
- Employment history, including duties and duration of all professional positions in QA;
- Professional activities, including allied affiliations and activities, published articles, awards, etc.;
- Education and training, including GCP, GLP, GMP and QA training.

Evidence of attendance at quality assurance meetings, such as the registration receipts, copies of your name badge showing the name of the meeting, etc. Acceptable QA meetings include those sponsored by SQA, SQA Regional Chapters or by other liaison or affiliate organizations. A syllabus must be attached for training sponsored by an organization other than SQA or any of its Chapters or Specialty Sections.

Proof of enrollment as a full-time student in an accredited college or university (minimum 12 credit hours). Submit one (1) of the following:
- Letter/signature of Department Chair/Advisor (see letter template);
or
- Signature/Information included on the hardcopy application;
or
- Formal verification document (i.e., enrollment verification form) issued by the college/university.
NOTE: Documentation is required every year with membership renewal.

EMCC will review your application and CV to confirm that you have an interest in the QA profession and that you qualify for Outreach membership. NOTE: Documentation is required every year with membership renewal.
