Requirements for each Membership Type 

In addition to dues payment, additional support documentation must be submitted as outlined in the 2017 Membership Brochure. Active Membership

Effective: 1 January 2017 - 31 December 2017 (dues are not prorated)

Documentation Required

Resume or Curriculum Vitae (CV) documenting at least two (2) consecutive years of experience in a research quality assurance role within (5) years immediately preceding the date of application.  Resume or CV must include the following: 

  • QA experience related to the audit functions to assure compliance with GLP, GCP or GMP as the Regulations/Standards/Principles relate to support of research activities.  FDA, EPA and OECD or other national/international equivalents are acceptable; 

  • Employment history, including duties and duration of all professional positions in QA;

  • Professional activities, including allied affiliations and activities, published articles, awards, etc.;

  • Education and training, including GCP, GLP, GMP and QA training.

and 

Attendance at one (1) recent (within past 5 years) QA meeting/training: Applicant must provide evidence of attendance by providing documentation such as a registration receipt, attendance certificate or copy of the badge showing the name of the meeting.  Acceptable meetings include those sponsored by SQA, SQA chapters or other allied research quality assurance organizations.  A detailed syllabus must be attached for training sponsored by an organization other than SQA or any of its chapters.

Written Sponsorship from two (2) Active SQA Members: Applicant shall provide written sponsorship from two (2) SQA Active members in good standing.  Letters of sponsorship shall be dated within one (1) year of the date of application.  Letter of Sponsorship Template  

Application Review 

All Active applications are reviewed by the SQA Ethics and Membership Credentials Committee and the SQA Board of Directors and take 4-6 weeks, from date all documentation is received, to be processed. 

  

 

Affiliate Membership

Student Membership

Proof of enrollment as a half-time student (minimum 12 credit hours) in an accredited college or university actively pursuing their undergraduate, post-graduate or doctoral degree, or be enrolled in school of professional studies related to research. Submit one (1) of the following:

  • Letter/signature of Department Chair/Advisor (see letter template); or

  • completed Student Verification area located on the hard copy application.

NOTE: Documentation is required every year with membership renewal.

All Student application are reviewed by the SQA Ethics and Membership Credential Committee and the SQA Board of Directors and take 4-6 weeks to be processed. 

  

 

Outreach Membership

Effective 1 January 2017 - 31 December 2017           

 

Application 

2017 Outreach Membership Application (PDF). The application must show that you are in a low or middle income country as defined by the World Bank.  However, if you work for a corporation that is able to support membership you may not be eligible for Outreach membership.

Resume or Curriculum Vitae (CV) documenting active professional involvement in research/regulatory quality assurance.  Activities may be demonstrated through employment history, professional activities, affiliations, published articles, or attendance at training or educational sessions.  These activities should be documented on the resume or CV.
