Refund Policy

Refund requests must be made in writing to SQA Headquarters at the following address.

Refunds will be made according to the following schedules.
 

SQA Fall Training     27 - 30 Spetember 2005

Cancellation requests received on or prior to 22 August 2005

 

Full refund of the Conference and Preconference Training registration fees, minus a $55 processing charge

Cancellation requests received
23 August 2005 - 2 September 2005

 

75% refund

Cancellation requests received
3 September 2005 - 12 September 2005

 

50% refund

Cancellation requests received after
12 September 2005

 

No refund

 

22nd SQA Annual Meeting     23 - 27 April 2006

A cancellation refund schedule for the Annual Meeting will be published in October 2005 when registration becomes available.

 

 

 

Substitution Policy

If you find that you are unable to attend the Preconference Training or the Conference, you may send a substitute in your place.  Each registration substitution will be charged a $55 processing fee.
 

Emergency Illness or Death of Registrant or Immediate Family Member

Attendees who are unable to attend the Preconference Training or the Conference based on a serious illness or death may receive a partial refund (amount paid minus a $55 processing charge) or full credit for a future meeting in the following instances:

  • Personal illness or death of the attendee; or
  • Illness or death in the immediate family of the attendee.

Requestors shall submit the request in writing to SQA Headquarters along with written documentation from a doctor.  The Program Chair shall review the request to ensure it is in accordance with written policy.
 

Returned Check Policy

Remittance must be made in U.S. dollars.  A $25 surcharge may be assessed to cover any collection fees.
